Fix many regression bugs introduced with v4.1
Support creating a new folder structure with hardlink file entries when using --action hardlink on disk folders
Any sequence of space will be reduced to a single space after formatting by default
{t} is now limited to 150 chars by default, thus should generally keep filenames below the 255 char limit
